BLUF: NIPT is a blood screening test that reads fragments of your baby’s DNA circulating in your bloodstream to assess the chance of a chromosomal condition.
During pregnancy, the placenta releases small fragments of your baby’s DNA into your blood. An NIPT test counts this cell-free fetal DNA to check whether any chromosome appears in a greater or smaller quantity than expected. An extra or missing chromosome produces conditions such as Trisomy 21, Trisomy 18, and Trisomy 13.
That counting method is why the test carries no procedural risk. Amniocentesis and chorionic villus sampling require an instrument to enter the uterus. NIPT reaches the same genetic material through a vein in your arm.
The result is a probability, not a verdict. Most women receive a low-chance result and need nothing further. A high chance result calls for confirmatory testing, because the American College of Obstetricians and Gynecologists states that cell-free DNA testing is not equivalent to diagnostic testing. These three trisomies carry the strongest evidence behind NIPT and form the core of every panel.
An extra copy of chromosome 21. Trisomy 18, Edwards syndrome, is an extra copy of chromosome 18. Trisomy 13, Patau syndrome, an extra copy of chromosome 13, and the rarest of the three.
Turner syndrome, a single X chromosome. Klinefelter syndrome, an additional X in a male pregnancy. Triple X syndrome, an additional X in a female pregnancy. XYY syndrome, an additional Y chromosome. Screening performance drops sharply in this group. In one published cohort, 41.7% of pregnancies flagged for a sex chromosome aneuploidy were confirmed on diagnostic testing, falling to 27.8% for Turner syndrome. A high chance result here always warrants confirmation.
The same sample detects whether Y chromosome material is present, which identifies your baby's sex from week 10. Ultrasound rarely gives a reliable reading before week 18.
Any pregnancy past week 10 is eligible for NIPT. Certain histories make the test more informative, and a few make it unsuitable.
ACOG advises offering chromosomal screening to all pregnant patients regardless of age, so eligibility is not limited to high-risk pregnancies. NIPT suits advanced maternal age, a previous pregnancy affected by a chromosomal condition, a family history of one, IVF or ICSI conception, twin pregnancies, and parents who want early information without a uterine procedure. Where clinical risk factors already exist, a high chance result is more likely to be genuine: positive predictive value for Trisomy 21 reaches 97.5%, against 85.7% in low-risk pregnancies.
Three situations call for a different approach. A vanishing twin produces false positives, so ACOG advises against screening there. An abnormality already seen on ultrasound calls for diagnostic testing and genetic counselling instead. And one screening method should be followed rather than several, which otherwise return contradictory risk estimates.
Two factors affect whether a result comes back at all. Higher body mass index and testing before week 10 both reduce the proportion of fetal DNA in your blood. An inconclusive result carries increased aneuploidy risk and needs assessment rather than an automatic repeat.
